Eligibility for Home Health Care
1) Patient recently hospitilized or discharged from surgery or illness
2) Patient with limited physical/mental capacities
3) Patient requiring medication teaching/compliance monitoring
4) Skilled care is medically neccessary as determined by the physician
5) Patient is homebound with the inability to leave home and/or leaving home requires assistance due to physical or mental condition
6) Patient with new or exacerbated diagnosis
